Know someone who needs better support?

You may be referring someone whose current support is not quite working, whose family is stretched thin or who simply needs the right people around them to move forward.

Share a few details through the form and we will take it from there. We will learn what is happening now, what the participant wants from their support and whether Nurture Your Way feels like the right match.

We look beyond the service list

A referral usually comes with more context than a name, phone number and requested support. There may be routines that keep falling apart, goals that have stalled, previous support that did not click or family members quietly carrying too much.

We take the time to understand that bigger picture. Our team listens closely, asks practical questions and looks for ways to make support useful in the participant’s actual day-to-day life.

For referrers, that means clear communication and confidence that the details you share will not disappear into a file. For participants, it means support shaped around who they are, what matters to them and where things could work better.

Frequently asked questions

Choosing a provider for someone else comes with questions. You may want to understand how quickly we respond, how we assess suitability and what communication looks like once the referral is submitted. We have answered the most common questions below so you know what to expect from the start.

How quickly will you respond after I submit an NDIS referral?

We will review the referral as soon as possible and contact you promptly to confirm it has been received. From there, we can clarify any missing details and discuss the next step.

We look at the participant’s support needs, goals, location, preferred routines and the type of relationship they are hoping to build with their support team. We also consider whether our experience and availability align with what they need.

The aim is to make sure the match feels comfortable and workable for everyone involved.

We review the information provided, contact the appropriate person and arrange a conversation to learn more about the participant.

This gives us a chance to understand what support is being requested, what has or has not worked before and what the participant would like everyday life to look like. We can then discuss suitable support options and the onboarding process.

This will depend on the instructions included in the referral. We can contact the referrer first, speak directly with the participant or connect with their chosen family member or representative.

We will always follow the participant’s preferences and respect any consent requirements.

Absolutely. You are welcome to contact us first to ask questions, discuss the situation and get a clearer sense of whether Nurture Your Way may suit the participant.
